To ensure Project Delta is the right fit for both the applicant and the service dog, our application process is designed to thoughtfully assess each individual’s readiness and ability to care for a dog. The full application includes information from the applicant, as well as input from family members or friends and the applicant’s physician.

Once your preliminary form is reviewed and approved, you will receive a complete application packet by mail. Submission of the final application packet requires a $25 check to cover the cost of a mandatory background check. This fee will be fully refunded upon acceptance into Project Delta’s Service Dog Program.
